Provide the name of the attendee, the EXACT date(s) of event(s) that you need a replacement or duplicate Certificate for (month/day/year), and how you would like to receive it. Once we verify your attendance, a duplicate Certificate will be generated.
The cost is $25.00 for each Certificate requested regardless of delivery method. Once a duplicate or replacement Certificate is prepared by AOTC to go out to you, the order is NON-REFUNDABLE.
